Plan Your First Visit

Arizona Center for Cancer Care wants to ensure you are prepared for your visit.

Anxiety is totally natural whether it is one’s first or fiftieth time visiting us.

Our entire staff not only understands your feelings – and those of family who may be with you – but wants to do everything in their power to make you feel as comfortable as possible.

It is best to arrive at least 15 minutes early to your first appointment should you need to complete any forms. You are welcome to bring family members or friends with you to the appointment.

What to Bring:
Be sure to bring the following during your first visit:

    • Your Insurance Card
    • A Government-Issued Photo ID (i.e. Driver’s License or Passport)
    • A list of all current medications – both prescription and over-the-counter – that you are taking
    • A list of any other vitamins or other supplements you are currently taking
    • A list of any existing medical conditions
    • A list of known allergies (environmental, food and seasonal)
    • A list of any recent medical procedures you have had
      Any known family medical history information
    • A list of any questions you – or your family – might have
    • A notepad, tablet, tape recorder or smart phone to jot down any notes from the visit, or to record any questions that might come to you while in the waiting room and/or immediately after the visit

Every effort will be made to explain your diagnosis and initial treatment plan and any associated processes during your first visit. Please plan on spending at least one hour to two hours with us as a result.

Be sure to make or verify your next appointment every time you visit the clinic. This includes lab, medication and radiology appointments.

Treatments such as surgery, chemotherapy and/or radiation do not take place during one’s first visit. This first appointment is more focused on meeting your nurses and members of your medical team, baseline tests that will help us to provide an accurate diagnosis and treatment plan moving forward, general medical evaluation and often a meeting with a benefits representative to review all billing and insurance information.

All of our locations are spread throughout the greater Phoenix area, offering convenient access to our services. If you're traveling from out of state, please remember that you will need to make lodging and travel arrangements to reach our facilities. We look forward to serving you and ensuring your journey to our Phoenix-based locations is seamless and rewarding.
